LUBInclusiON – preventive and independent measures in the local environment and the creation of new places of social services

The purpose of P shall be: — prevention of institutionalisation and social exclusion, – support of 266 people. (including OS, OZN, OzN. with mental disorders or mental impairment, dependants) in a friendly local environment – prolongation of social activity OS – preventing the social exclusion of people with mental disorders or mental impairment, who well predict to be independent The participants of the project are people at risk of marginalisation. The provision of well-designed social services tailored to the needs of the OS and OZN aims to prevent the process of social exclusion. The environmental aid scheme is an alternative to costly round-the-clock care and keeps active and self-reliant at the current level. The applicant has planned a comprehensive support system to combat exclusion and social isolation: — social services (participation in KS, various occupational therapies, thematic workshops, activities aimed at social and cultural activation OS working with seniors according to the concept of educator. M. Montessori), – support offered to residents of assisted housing focusing on restoring the ability to function properly in society, developing the skills and habits necessary in everyday life, professional activation, rational management of own budget, as well as self-control of health and mental health, – supporting day-care facilities for seniors and round-the-clock care facilities (up to 30 people), – implementing integrated social services of a preventive nature and by activating day-care facilities for seniors and round-the-clock care facilities (up to 30 persons), – the implementation of integrated social services of a preventive nature, and by activating equipment for senior citizens (up to 30 people).
