Autumn of life – in good condition without pain 2

The project was developed in partnership with the Strzelce Municipality Op. (Social Assistance Centre). The project covered all the districts of Opolskie voivodship. The need to implement the project results from the demographic situation in the Opolskie voivodship, i.e. the increase in the number of people in the area. 60 years of age, including those at risk of poverty or social exclusion and/or disability, (according to GUS data on 31.12.2016r. in Opolskie voivodship number of people > 60+ was 240,855 and were mostly women – 58.26 %), who are often cared for by family members and those who do not require care of third parties, but who have cardiovascular diseases, apnoea, pain syndromes, and do not have access to high-quality health, rehabilitation and nursing services, improving their health status, functionality and reducing premature mortality. The tasks planned in the project aim to eliminate the above barriers. The project envisages tasks: the separation of 500 elderly people (project participants) at risk of poverty or social exclusion (reliable persons), provision of home care in the form of neighbourhood care services – in addition to support under tele-care (18 people), telecare for 500 participants of the project, rehabilitation services for min. 300 participants of the project and provision of transport for medical tele-care services for 260 participants of the project. The project will contribute to achieving the main objective of enabling access to high-quality health services for elderly people at risk of social exclusion, thereby giving them the chance to extend and improve their quality of life, to live without pain with greater physical fitness.
