The subject of the project is to take measures to improve the health of people at risk of poverty and social exclusion from the Korfantów commune and the surrounding areas: the elderly and the disabled. The project will provide health services in the home environment and in the Opolskie Centre for Rehabilitation in a daily form. A rental of rehabilitation equipment will be organised and an educational activity will be organised: training for carers in the field of medical care and medical staff – in order to improve competences and improve the quality of services provided. The project will prefer people with medical indications directly or up to six months after hospitalisation, fulfilling the conditions for recruitment. 10 places of care will be created in the daily form of stay, and up to 6 patients per day will be commuted by OCR staff (outbound group will be activated). The project responds to the perceived social problems: —accelerated process of ageing population (increased number of single-person households living alone with difficult access to health services) and the problem of increasing the economic burden of families, -difficulty or inability to perform basic life activities independently (according to the Barthel scale) and the need to take measures to improve disabled persons, especially at 65+, -small coverage of the needs of older residents in terms of living places in daily homes. As a result of the project, nursing and medical assistance will be intensified, including rehabilitation and physiotherapy, occupational therapy, activities will be carried out, leading to improvement and increased comfort of life of these people. Patients will be provided free of charge, comprehensive health services, transport and food.
