The main objective of the project is to increase accessibility to social services, in particular environmental services, care services, telecare services for people at risk of poverty or social exclusion by supporting and improving the daily functioning of 80 people. (55K and 25M) dependants living within the meaning of the Civil Code within the territory of the Tuszów National Municipality and enabling them to continue to function in their existing environment, despite the limitations experienced in meeting the basic and necessary needs and barriers to integration into the environment. Through adequate support provided by dependants, these restrictions and barriers will be overcome, contribute to maintaining or raising their standard of living through the establishment and operation of a daily assistance home, increasing access and delivery of care services and providing modern 24h teloopieca between 11.2017 and 03.2019. Main results: The main objective of the project will be achieved by implementing individualised and comprehensive support provided in a differentiated form. In addition, the inclusion of the Participants in the abovementioned services will contribute to: — the number of social services supported by the programme after the completion of the project – increase the quality and comfort of life – increase access to care services – increase access to care services – increase access to care services – increase access to care services – increase access to specialised care services.
