Gdański Program for Prevention of Diabetes – Health Policy Programme on Prevention of Diabetes Type 2 in people of working age in the City of Gdańsk and the municipality of Stegna

The project is consistent with the identified (ZIT) initiative and adopted “Regional Health Policy Programme for Prevention of Diabetes Type 2” (RPPZ), positively opinioned by the Agency for Medical Technology Assessment and Tarification – (opinion of the President of AOTMiT No. 290/2017 of 16.10.2017) and adopted by the Voivodeship Local Government. It’s the Pomeranian. The project will consist of a process of population health intervention in order to reduce the incidence (prevalence) of diabetes type 2 inhabitants with undiagnosed type 2 diabetes from the area of action, active age, at higher risk in particular those aged 35-64 (i.e. people aged 35-45 years if FINDRISK achieved 12 points and more, and at the age of 45-64 years, regardless of the results in the above-mentioned surveys). The project will be implemented in several interconnected stages, including including: (1) Qualification of people at risk of type 2 diabetes (preliminary assessment of criteria for inclusion by means of the FINDRISK survey) in educational activities, (2) conducting oGTT research to identify people with diabetes, pre-diabetic condition and to verify the effectiveness of health interventions, (3) conducting an educational programme in the form of a complex medical-behavioral and educational intervention process carried out by an interdisciplinary team of specialists: doctors, nurses, nutritionists, psychologists and physiotherapists. The project will develop medical and intervention procedures and dedicated educational materials in both traditional forms (printed, intended for direct meetings, stationary meetings) as well as electronic access to e-learning and instructional materials (audio/video) intended for use on mobile devices (laptop, phone, tablet, etc.).
