The aim of the project is to reduce the number of people and families at risk of social exclusion in Gdańsk by developing and implementing community services in 5 Centres of Social Activation (CAS Orunia,CAS Nowy Port,CAS Stogi,CAS Dolne Miasto,CAS Biskupia Górka) located in Gdańsk areas with a high risk of poverty and social exclusion in the period 2017-2022. The target group is in particular: children and youth from dysfunctional families and their caregivers/families experiencing difficulties in fulfilling the functions of caregivers.,persons attending/leaving the foster care and their caregivers, dependent/disabled persons and their caregivers.Pref.participation of persons benefiting from the POPŻ. Despite being located in areas of above-average social exclusion, the CAS offer services to all Gdańsk patients. The support of participants will be decentralised, its main burden will be transferred to districts in line with the need to provide individualised and comprehensive services in the local environment. The district service system will be implemented in the form of a Centre for Social Activation and distributed services in the district. All participants will be covered by a comprehensive diagnosis (in 7 areas) on which reintegration paths will be developed. Each participant will have the opportunity to use the services he needs most, e.g. mental support, guidance, care services, stay in a day care facility. Each participant will be assisted (assistant/assistant of the family). The project is in line with the ZIT strategy and uses environmental animation and volunteering. The project will result in support of at least 25 permanent places of service to the community. As part of the project, 64 people at risk of poverty or social exclusion will seek work, participate in education, training, qualifications, work after the end of the project.
