The project is directed to 174 people living in Pow.Lęb. at risk of social exclusion and their families belonging to the following group (according to the executioners of the activities): +Association of Friends of Persons with Disabilities at the SOSW in Lębork (20U): 1.Person with intellectual disabilities, in particular SOSW graduates and remain passive.profession.(20-45 y.r.)-20U(10K,10M) +Division Lęb.(50U): 2.Personally severely sick-requiring palliative care (non-medical)-20U(10K,10M), their care and families-30U(15K,15M) +PCPR in Lęb.- within the scope of the replacement care system.(100U): 3.(family,institution)-0Y-25Y-54U(27K,27M) 4.Replacement carers(administering RDD,professional families,non-professional,related)and candidates-36U(19K,17M) 5. 7.Os.s.little pass and leave the garrison seal(16-25Y)-20U(10K,10M) of which 16Uw/infor each of these above mentioned reintegration path.
