The aim of the project is to facilitate access and increase the availability and quality of social services preventing poverty and social exclusion through the development of care services and specialised care services, including: operated at the place of residence of an independent person, support of actual caregivers, specialised nutrition, removal of architectural barriers, loan of rehabilitation beds, care services based on modern technologies. The target group will consist of 100 people excluded or at risk of poverty or social exclusion, inactive and dependent on age, i.e. 60+, including: persons with a disability ruling, persons with mental disorders, benefiting from PZ and others. The project creating an environmental support formula will be implemented in the period from 01.07.2018 to 31.12.2020 and will be addressed to the target game and their carers, inhabitants of the nutritional area.
