The main objective of the project is to create a comprehensive system of support for dependants, in particular older people over 65 years of age from the City of Leszno in the period from 01.07.2020 to 30.06.2023Y. within the framework of the Environmental Centre for the Support of Older Persons (SCWOS) consisting of providing services within the framework of: Daily Healthcare Home (DDOM), Home Hospice (HD), Supported Housing (MW), Senior Daily Home and 3 Senior Clubs (activities included in SCWOS carried out out-of-project), care services in the place of residence (UO), nursing home care (POD). The project will be implemented in accordance with the scheme of support for older people. It will be complementary to the project 9.1.2 Construction of the Leszczyński Senior Services Centre. Target group of at least 219 dependants, in particular older women (159) and men (60) from the area of Leszno. The actions included in the project are aimed at reducing the number of people and the duration of medically unjustified hospitalisations. The forms of support proposed in the project will be equally open to both sexes. Partners will endeavour to ensure that actions are free of stereotypes attributed to gender roles. The project will result in a single supported place for the provision of social and health services, which will exist after the completion of the project; at least 219 people at risk of poverty or social exclusion covered by social and health services.
