Main objective: Assessing the quality of therapeutic rehabilitation by harmonising the patient’s functional assessment using the International Classification of Functions, Disability and Health (ICF). The project will develop functional evaluation standards that will allow for a uniform description of patients (categorised profile) who benefit from rehabilitation in a stationary as well as outpatient form. Developed and implemented standards will allow the introduction of a uniform system of planning patient support (intervention card).Assessment of the effectiveness of the planned support (post-rehabilitation evaluation card) will allow for a comprehensive assessment of the quality and effectiveness of the support provided both at the level of a particular person, as well as at the level of the hospital as well as all participants in the rehabilitation process. The full implementation of this solution will allow to assess the effectiveness of spending public funds and rationally plan preventive actions and actions related to patient care. The main tasks in the project include: 1) preparation of a new solution in cooperation with the transnational partner, 2) testing of developed solutions, 3) analysis of the effects of tested solutions together with the transnational partner and improvement/modification of developed solutions in order to verify and improve the developed tools and procedures, taking into account the data obtained during the analysis of the test phase, 4) implementation of a new solution into practice,5) developing recommendations (instructions describing the elements necessary for the functioning of the developed solution in relation to the requirements e.g. organizational, technical, competence-care, cost).
