The main purpose of the project is to increase the detection of diabetes among 2.700 inhabitants (1.620K, 1.080M) in Warsaw and the sochaczewski district by conducting educational activities in the field of diabetology and screening for diabetic detection by 31.12.2021. The planned tasks are in line with the objectives of the Regional Health Programme of the Mazowieckie Regional Government and include: information action (Module I), medical consultations and educational activities (Module II) and training of medical staff (Module III). The main PARTIES OF PROJ. covered by direct support are people area. 60 years of age living in Warsaw and the area of sochaczewski not yet diagnosed in the direction of diabetes, who will sign a print of informed consent to participate in the project. In addition, doctors/nurses, POZ will also take action. EXPECTED RESULT INDICATOR: number of post-project health service providers supported in the programme. ESTABLISHED WSK.PRODUCT: number of people at risk of poverty or social exclusion covered by health services in the programme. The other indicators specified in the RP will be shown and reported in accordance with §30(3) of the model flat-rate contract for the competition in question.
