Cardiovascular disease is a chronic disease that usually develops throughout a lifetime and usually progresses to an advanced stage until clinical symptoms occur. These diseases are considered work-related diseases as they affect a large working population and occupational hazards and working conditions are one of the possible risk factors influencing the deterioration of health. As part of the project, it is planned to carry out information and promotion activities aimed at recruiting participants for the project and implementing rehabilitation measures (ambulatory cardiological rehabilitation) for people with cardiovascular diseases in order to take up or continue their professional activity. The project will cover people living within the meaning of the CC in Opolskie voivodship between the age of 18 and reaching retirement age – rehabilitation measures and medical staff – training aimed at improving qualifications. Priority will be given to workers at risk of premature exit from the labour market due to health factors caused by working conditions. The project will cover the following phases: 1) information and promotion activities, 2) recruitment and qualification of participants for the Programme, 3) implementation of the Programme, 4) monitoring and evaluation, 5) training. Based on the results of the qualification, a specific group of residents aged 18-65 will receive multi-component care. Each participant will be under the control of a specialist team of doctors. The participants of the project, based on the recommendations of doctors, will have the opportunity to participate in various educational workshops, physical activity classes and vocational rehabilitation classes. The project will be implemented in accordance with the Regional Health Programme. Project activities will not replace health care services financed from public funds and will be carried out in accordance with the provisions applicable to the above-mentioned benefits.
