Support programme for elderly and disabled people from the communes of Korfant and Łambinowice.

The aim of the project is to take measures to improve the health status of people at risk of poverty and social exclusion from the communes of Korfant and Łambinowice: elderly and disabled people. As part of the project, health services will be provided in day-to-day medical care centres for the elderly established at the Opole Rehabilitation Centre. Participants of the project will be able to use the rental of rehabilitation equipment retrofitted as part of the project already carried out. In the project, people with a medical indication will be preferred either directly or up to half a year after the hospitalisation, meeting the conditions for recruitment. 4 places of care will be created in the daily form of stay. The project responds to the perceived social problems: —the accelerated ageing process (increase in the number of single-person households of older people living alone with difficult access to health services) and the problem of increasing the economic burden on families, – the difficulty or inability to carry out basic life activities independently (as indicated by the Barthel scale) and the need for improvement measures for people with disabilities and dependent persons, especially at the age of 65+, – the small extent of securing the needs of older residents in terms of places of residence in daily homes, with a systematic increase in their number. As a result of the project, nursing and medical assistance will be stepped up as a continuation of hospital or outpatient treatment, including rehabilitation and physiotherapy, occupational therapy, and activities leading to improving and improving the well-being of these people. The project participants will be provided with free, comprehensive health services, transport to the place of health services, meals.
