Civilisation diseases, including above all excessive weight and diabetes, are one of the most common lifestyle problems in Poland, including the Opolskie voivodship. The project planned to carry out a comprehensive education and health intervention and to implement measures to prevent overweight, obesity and diabetes. The project will cover persons living within the meaning of the CC in the Opole district and the city of Opole. Based on the results of the screening, a specific group of residents aged 6-65 will receive multi-component care during the 2-year period. Each participant will be under the control of a team of doctors (multidisciplinary team). The participants of the project, based on the recommendations of doctors, will have the opportunity to participate in various types of physical activity classes and activities in the field of healthy eating. During the implementation of changes in dietary and health attitudes, each participant will have the support of a psychologist/therapist. At the same time, a number of educational information measures for the general population, combined with physical activity, are envisaged. In order to implement the project properly, the Council will be responsible for the substantive quality of the activities carried out (including compliance with medical procedures). During the implementation of the project, in order to reach the widest possible group of residents, the Applicant plans to establish cooperation with centres/entities such as: Pos, OPS, PCPR, PLO and NGOs. The project will be implemented in accordance with the Regional Health Programme. Any activity funded by the project will not replace healthcare services, the funding of which is guaranteed from public funds.
