The project “Sopot – Active residents (Stage 2)” is part of the Metropolitan System of Social and Professional Activation. It shall aim at increasing the socio-occupational participation of people at risk of poverty or social exclusion, including in particular persons with disabilities, through the implementation of a reintegration pathway using active inclusion services of a social, occupational and educational nature. As a result of the implementation of the project activities, a reintegration path will be planned for each participant using personalised forms of support to strengthen social competences and equip them with the competences and qualifications needed on the labour market. The planned actions include, in particular: — Comprehensive diagnosis carried out at the initial stage of the project by a team of specialists employed in the project. The diagnosis will be carried out in 7 areas of operation of the project recipient: psychological, residential, family, social, social, health and professional. The needs, capabilities, resources and constraints of project participants will be identified. — The reintegration path for project participants – its creation, implementation, monitoring and evaluation will be the responsibility of the assistant recipient, who will also act as a job trainer in the project, supporting the search and maintenance of employment. — Psychological counselling – individual and group counselling (depending on the needs of project participants) carried out by a psychologist, – Individual and group counselling provided by a career counselor, – Training and courses (upgrading qualifications or competences) – after determining professional abilities, to suit the individual needs and capabilities of the participants, – In addition, each participant will be included in additional activities that will result from the individual needs of the recipients.
