Cross-sectoral partnership project 12P-10 JST,1 PES,1 NGO allowing 710 GD (468K 242M) of which 590 persons(438Sen and 152ON) are not themselves, zagreb social and poverty and 120 their carers have access to high-quality tailored and individualised social services/US The creation of 124 permanent witness places deinstitutionalised US in the district of Bytów-telecare, home rehabilitation, psychotherapy, neighbourhood, assortment, animations-corresponding to the actual and forecasted need from the local diagnosis of US/LD demand adopted by all P. According to the Implementation Strategy of US 9 municipalities will create an integrated local US system (US points in OPS/PCPR resource) using modern, remote technologies, animation instruments of the center and promotion of volunteering. The scope of support will be extended to include health prevention, integration and activation of community premises. Trainings, workshops, recreational and animation classes will be conducted. The department in the project implements b. close to GD. Permanent places of the US witness in the smallest villages of the county.The project will allow GD access to knowledge, skills, caregiver benefits, rehab equipment (utw. rental) and animation (utw.25 square anim). This will raise the standard of living of GD from rural areas. The activity of OZ will be strengthened by the formation of Senior Councils (associated councils-consult bodies) and the Wolontar Wheel. This will allow for a long-term planning department for the OZ centre. The wide impact of the project is possible thanks to the cooperation of three sectors. Durability and efficiency is to guarantee co-operation between entities and units of the whole district. Social activities carried out in the project are a multicolored range of care services (anal to the colorful range of Spanish dancers), which is designed to respond to the individual needs of OZ and bring a change combined with friendship and kindness. The most important objective of these activities is HUMAN under threat of malpractice and its improvement, prevention and animation. The seniority title is intended to introduce lightness, a sense of joy and a positive attitude in working with OZ
