The long-term economic crisis has led to an exacerbation of social and economic problems in Athens and the risk of social exclusion is intensified for a large part of its inhabitants. It is currently estimated, based on the data of the Municipality, that more than 70,000 residents of Athens are threatened with social exclusion. The project aims to provide one stop shops with combined medical and social/counselling services adapted to the new conditions for the residents of Athens who belong to the vulnerable groups in Athens. The services provided include, but not limited to: Primary health care services including integrated and continuing health care services to elderly and chronic sufferers with polymorbidities; Psychosocial support for individuals or families in trouble; Support for professional integration; Primary prevention, education and health promotion; Information on benefits and support to which they are entitled; Legal support. Legal support. In the framework of the project, 4 multipurpose centres will operate in the first phase: Solonos (1st DK) (b) Petralona (3rd DK) (c) New World (2nd DK) (d) Kypselis (6th DK).
