The purpose of this project, lasting 24 months, is the interconnection of the Memory Clinic and Mental Functions of the General Hospital “Papanikolaus” of Thessaloniki with Primary Health Care structures aiming at the single and coordinated treatment of patients. The ageing of the population (with the increase in the rate of dementia) and the increasing decline of mental functions at younger ages (which are largely undiagnosed) have a serious impact on the functionality and mental health of sufferers, as well as on the performance of the daily activities of patients and their caregivers. The holistic treatment of patients, based on the principles of psychosocial rehabilitation is of the utmost importance. A disorder of cognitive function or neurocognitive deficit occurs primarily in neurodegenerative diseases (Alzheimer’s disease, Parkinson’s disease, Parkinson’s syndromes) and aging (Ageneration). Deficits, however, arise very often from substance use, cranio-brain injury and other neurological conditions that occur at younger ages, such as Multiple Sclerosis, Vascular Brain Episodes) and psychiatric disorders (psychotic and emotional). This group of young people with mental deficits needs special care and interdisciplinary treatment and therefore it becomes necessary to operate the Single Clinic for Memory and Mental Disorders provided in the framework of this project.
