The aim of the project is to improve access to health promotion and disease prevention services in Jelgava city, especially the local community exposed to the risk of poverty and social exclusion, by implementing a complex to change health habits in 2017-2023.The target group of the project is inhabitants of Jelgava city, especially groups at risk of social exclusion and poverty: The poor and the low-income population; Unemployed persons; Persons with disabilities; People over 54 years of age •children.The main activities of the project until June 30, 2020: 1) the implementation of health promotion measures for the local community and target groups; 2.implementation of disease prevention measures for the local community and target groups; 3) project management; 4) Ensuring the implementation of the project; 5)Implementation of information and publicity measures.The main activities of the project from June 30, 2020: 1) the implementation of health promotion and disease prevention measures for the local community and target groups; 3) project management; 5)Implementation of information and publicity measures.The planned results of the project: •5946 citizens belonging to the particularly supported target groups and participating in health promotion and disease prevention measures within the project; •organised 3476 health promotion and disease prevention measures.Total project costs: EUR 1071153, of which EUR 910480,05 (85 %) – European Social Fund funding and EUR 160672.95 (15 %) – state budget funding.The planned duration of the project: 79 months – from May 2017 to December 31, 2023.
