The project aims to strengthen and develop an innovative model of integrated services dedicated to elderly people with different vulnerabilities and dependents, to promote the concept of active aging – as a long-term solution to the demographic aging process. The project offers a concrete and innovative model of promoting active ageing by integrating older people into society, providing social and medical services for them according to the degree of dependence, improving the quality of life and ensuring a decent life for the elderly in Bacau. General objective of the project CONTRIBUIE TO THE SPECIFIC OBJECTIVE OF the call OF PROIECTS 4.4, which aims to “reduce the number of persons belonging to vulnerable groups by providing social/medical/socio-professional/professional/training services appropriate to specific needs”, through the integrated approach of the package of socio-medical services that will be provided within the project and which will directly contribute to the reduction of the number of older people included within vulnerable groups. The project is also linked to Investment Priority 9.ii, which aims to “Increase access to accessible, sustainable and high-quality services, including healthcare and social services of general interest”. Moreover, through its objectives, activities and results, the project attaches great importance to overcoming the vulnerability situation of elderly people with different degrees of dependence, contributing to the achievement of the main expected outcome of the call for projects, namely “Number of persons belonging to vulnerable groups (old people) who have overcome the vulnerability situation by providing social/medical/socio-professional/vocational/training services appropriate to specific needs”. The general objective and specific objectives of the project are built based on the extensive expertise of over 2 decades acquired by the applicant in integrated programs for the provision of social and socio-medical services addressed to the elderly with all degrees of dependence. At the same time, the activities proposed within the project are a response to the real needs of the elderly in Bacau, as they were highlighted in the analysis carried out, through questionnaires applied to potential beneficiaries, visits at home, as well as from the analysis of the available secondary data. Thus, the package of integrated socio-medical services proposed to be created and developed within the project that will benefit the 200 elderly people is directly anchored to the reality in Bacau and correlated with the needs of the elderly.
