OG: Reducing the number of people at risk of poverty and social exclusion from the Poienesti community and revitalising community relations, by operationalising and sustaining a continuum of integrated services and measures, implemented by multidisciplinary teams, offered through the Poienesti Integrated Community Assistance Center (which will act as a multifunctional centre), as fixed and mobile services, addressed to the entire community. The general objective of the project responds directly to the achievement of the specific objective of the program and call, by addressing the needs of vulnerable people, affected by persistent poverty, as well as categories on which the various dimensions of exclusion tend to perpetuate from one generation to the next. The ex-ante analysis carried out at community level revealed that socio-economic exclusion is perpetuated when poor learning outcomes and poor health significantly limit the opportunities for access to the labour market for the next generation. Also, the relevant experience of the project partnership has shown that breaking the vicious circle of poverty and exclusion requires individualised interventions, designed to address the underlying factors of inequality. In order to support this approach, the project aims to develop and operationalise the model of Poienesti-CCAIP Integrated Community Assistance Centre, as well as the development of mobile, multidiscipline teams to facilitate access to integrated services for all members of the community, directly participating in the achievement of priority axis 5 – Local development placed under the responsibility of the community, Thematic Objective 9: Promoting social inclusion, combating poverty and any form of discrimination, Investment Priority 9.vi: Local development strategies placed under the responsibility of the community Specific objective 5.2: Reducing the number of people at risk of poverty or social exclusion from marginalised communities in rural areas and cities with a population of up to 20,000 inhabitants by implementing measures/operations integrated in the context of CLLD mechanism. The project emphasises the essential need for coordination and complementarity of social, educational, health and health services and employment services, in order to facilitate the employment of people who are not working, but are fit for work, and to strengthen community relations and help increase community resilience. This approach to poverty and social exclusion is based on the concept of providing integrated services and on ensuring harmonisation and coherence between different programmes and interventions, both at individual/household level and at community level.
