Healthy Communities 2B

The main objective of the project is the implementation and development of medical mediation for residents and members of CLLD. The main activity of the project is the implementation and development of medical mediation, education in CLLD. It is an activity based on the long-term successful practice of so-called health mediation, which engaged and educated laypersons from target communities to carry out systematic work at community level, with programme emphasis mainly on prevention and assistance with bridging barriers in access to health care services. The project continuously builds on the currently implemented and ending project NP ZK 2A. The main activity will be implemented through Health Support Assistants (hereinafter referred to as “APAs”) who will deal with educational awareness-raising activities aimed at improving health literacy, personal and communal hygiene levels, preventive activities focused on postpartum care, breastfeeding, preventive examinations, compulsory vaccination, identification of infectious diseases and other assistance activities aimed at improving relations and communication between the population of the CLLD and healthcare providers. These activities are currently also carried out in the project ZK 2A and the project ZK 2B will ensure their smooth implementation by the end of 2022. At the same time, the activities of the ZK 2B project will be extended to include interventions focusing on other key local social determinants of health (hereinafter referred to as “the CSD”), e.g. unsatisfactory material conditions, the enormous psychological burden on the population there, barriers to access to care services and all the directly related aspects of social exclusion of the CLLD population. The target group is persons from the MRK environment (participants of the project) to whom services will be provided mainly in the settlements of MRK in seven regions of Slovakia (except for the Bratislava region).Measurable indicators of the project will monitor the number of persons from the target group and the number of APZs that will provide services to the target group.
