The main objective of the project is to build systematic provision of health promotion services through a network of workers at community and regional level. The project will be implemented through a main activity, which is the implementation and development of health mediation with a focus on promoting social inclusion and 8 sub-activities within a more developed region.  Compared to the first phase (NP ZK 3A), the present project develops and innovates activities with development elements that have proved successful in realising activities on the ground even outside the more developed region. These include, in particular, the extension of methodologies, more targeted settings for field activities, new jobs directly in hospitals, as well as a change in the title of the job. As part of the main activity, the project aims to prepare human resources for emergency, emergency and crisis situations, especially in the event of epidemics and pandemics (in the context of the Covid 19 pandemic). The expected benefits of the project activities are the promotion and development of health-educational education programmes for residents of CLLD and other vulnerable groups. Measurable indicators of the project sú:Počet those who have used new, innovative services or measures to implement social inclusion servicesNumber of projects targeting public administrations or social services at national, regional and local levelNumber of activities aimed at raising awareness of the rights of persons with disabilities
