The telemedicine integration project associated with the monitoring of chronic wounds in the community of St. Martin is unique in the Caribbean in both its approach and its organisation. It will make it possible to develop, implant, experiment and begin the sustainability of remote medical services for a precarious population living in a territory hit by double insularity. The reasons for the project are the harmonisation of the management of complex wounds, the creation of the city-hospital link, the speed of diagnosis and care thanks to the New Technologies, the forecasting of a prognosis of healing, the comfort of the patient and the ambition to deploy an Inter Caribbean project. In addition, the R2C project is the operational delineation of the Regional Telemedicine Program (RTP) under the “Management of Chronic Diseases” component. Teleconsultations/tele-assistances and specialised tele-expertises make it possible to better deal with patients with complex diseases at home, by preventing them from travelling, unjustified hospitalisations and reducing unequal access to care in isolated areas. The wound expert site will be located at CH Louis Constant Fleming de Saint-Martin and the applicant sites will be located in Saint-Martin (HAD, EHPAD, liberal IDE, attending physician,...)
