The project aims to create conditions for a sustainable transition from institutional care to the system of services provided in the family and community in Lithuania. Target groups of the project: children without parents (including infants), children at risk and their families; children with disabilities and young people with intellectual, mental disabilities and their families; disabled adults with mental or mental disabilities and their families. During the project period, it is planned to develop a methodological framework for new forms of community-based services for children, persons with disabilities and their families (carers, carers) and to improve the quality and accessibility of existing social services, to carry out an assessment of the individual needs and motivation of the staff of pilot care institutions, to improve the professional competence of employees who administer social services and provide services to children, persons with disabilities and their families (guardians, carers), to carry out studies on monitoring and evaluation of institutional care reform and interinstitutional; inter-agency and cross-sectoral cooperation for children, persons with disabilities and their families (guardians, carers) in the areas of developing new forms of community-based services, ensuring the complexity of providing social services and drawing up plans for reorganised institutions, community services and their infrastructure, as well as to promote the care/care of children of natural persons, families and non-governmental organisations, adoption of children and/or social services for the rest of them without parental care, for those exposed to social risks, persons with disabilities and voluntary work in this field, changing the values of society and the local community for the transformation of institutional care, ensuring publicity of ongoing transition processes, visibility of the project and results, and ensuring the quality of the outcomes of the institutional care transition, and carrying out coordinated management, monitoring and evaluation of the transition reform. The project is carried out throughout Lithuania.
