In order to optimise the Lithuanian health care system by strengthening the primary health care chain, and to provide high-quality and timely personal healthcare services to patients with two or more chronic non-infectious diseases, the implementation of the provisions of priority measure No 08.4.2-ESFA-K-616 of the 2014-2020 Operational Programme for investments of European Union funds is launching the project “Continued Health Care for chronic patients (TELELISPA)”. The model planned for the project is based on the work carried out under the EU-funded project “Application of Good Practices for Chronic Diseases” (CHRODIS PLUS). The model to be tested by the project promoter and partners covers all sections IV and most of their components, the scope of which was lower in the CHRODIS PLUS project. The new model will be adapted and improved, taking into account the problems, results and unrealised challenges encountered during the CHRODIS PLUS testing process, such as: teleconsultation and telemedicine development, reducing the gap between rural and urban health care services, increasing patient self-care, increasing the participation of CSP team members in providing services and developing their competences. In order to test the innovative model on the widest possible scale and to evaluate the results of the quality and accessibility of the services provided by the model, the CSP centres for urban, district and rural areas are selected as partners. It is planned that about 385 patients will participate in the piloted model during the project implementation. The project will pursue the objective set out in point 17 of Annex 1 to the Healthy Ageing Action Plan, the objective set out in point 19 and implement the measure provided for in point 30.6. The main objective of the project is to improve the quality and accessibility of personal health care for patients with two or more chronic non-infectious diseases.
