Integrating the integrated healthcare model into primary care for polypathic patients

Currently, in clinical practice, there is a single-disease-oriented healthcare model, which puts polysickens patients exposed to fragmented care, which results in insufficient, inefficient, inefficient care for polyligot patients. Polyligotum is associated with complex healthcare and polypharmaceuticals, which increases the potential risk of adverse reactions and drug interactions, the possible misuse of medicinal products or the administration of inconsistency drugs. The development of multi-patient healthcare models is becoming a priority in most healthcare systems that are still focused on acute rather than chronic diseases, and it is therefore necessary to develop an integrated model of care for polypathic patients, based on the principles of teamwork, which improves the quality of care for polypathic patients. Primary care for polyligotic patients needs to be coordinated and managed in order to reduce the risk of inappropriate treatment. One of the most widely tested models of care for two or more chronic patients in Europe is CHRODIS, CHRODIS PLUS. Santaros clinics have made a significant contribution to the implementation of these projects. The specialists of Santaros Clinics have unique experience in the development and testing of this model, so this model has been chosen to test it on a wider scale. The model will be tested in 4 Lithuanian primary health care centres in Druskininkai, Pakruojis, Aukštadvaris and Vilnius. The effectiveness of the components of the chosen model is assessed and known, but there is no data on how such a model would work in several healthcare facilities of different sizes, with between one and a half and a dozen thousands of registered patients. The results of the project are expected to lead to improvements in healthcare for patients with two or more diseases, and will allow for a more efficient use of healthcare resources.
