The main problem addressed by the project is that people with mental and/or mental disabilities in the Vilnius region (hereinafter referred to as the target group) do not have access to personalised services and necessary assistance, to engage in community life and to participate in it without social exclusion. The project aims to address the problem, i.e. to create the conditions necessary for an effective and sustainable transition from institutional care to community-based services that promote ownership and full participation in the community. 9 partners have been involved to achieve the project’s objective. For the implementation of the project activities, the following will be carried out: Construction of 6 group living houses and purchase of furniture/equipment (computer 6); 1 construction of independent living houses and purchase of furniture/equipment; 2 Acquisition/repair of sheltered housing in Lazdynai and Šeškinė elderships and purchase of furniture/equipment (computer 2); Construction of 2 day employment centre/social workshops and purchase of furniture/equipment; 2 social workshops and purchase of furniture/equipment; 1 day center construction and purchase of furniture/equipment. After the implementation of the project activities, the infrastructure for community-based accommodation and day-to-day employment services in Vilnius region will be created.
