The project is part of the overall process of enabling independent living for people with mental disorders and mental disabilities by providing quality and accessible cross-sectoral services of resident type and according to the individual needs of individuals. It is implemented in implementation of the measures set out in the Action Plan 2018-2021 for the implementation of the National Strategy for Long-term Care. It will provide preparation for the provision of services in the community, after an appropriate infrastructure has been built with funds under the Operational Programme Regions in Growth. A Methodology for carrying out individual needs assessments and plans for support of persons with disabilities will be developed; Methodology for closing homes for people with disabilities and for reforming old people’s homes. 22 assessment teams will be set up to carry out individual needs assessments and plans to support more than 700 persons from 10 specialised institutions designated for closure and more than 200 long-term residents of state psychiatric hospitals on social indications. Training materials for the training of social and health system professionals providing social and integrated health and social services will be prepared. The assessment teams and the staff of the institutions designated for closure will receive support through supervision. There will be plans for closure of the 10 specialised institutions. An analysis of the remaining 151 institutions for people with disabilities and the elderly will be carried out, which will be the basis for the development of the second Action Plan for the implementation of the National Strategy for Long-term Care. The users of the institutions will be actively working to prepare for their removal and relocation to the new services. A Communication Strategy will be implemented, the main purpose of which is to change the attitudes of society towards people with mental disorders and mental retardation.
