The project foresees the provision of hourly mobile integrated health and social services in the home environment on the territory of the Municipality of Byala Slatina. The services will be aimed at persons with disabilities and elderly people who are in complete or partial inability to self-service at home, improve their access to basic social and health services and achieve independence and social integration. This will protect the public interest and meet the needs of the population. The project proposal combines various actions to overcome isolation and ensure integrated access to health and social services. The main need, which will be met through the implementation of the project, is the provision of complex care to improve the quality of life of older people and on the other hand to enable unemployed people of working age to be involved in the provision of the new services.The project groups are:- People with disabilities and their families;- Adults over 65 years old with restrictions or inability to self-service. The main objective of the project is to improve the quality of life and the opportunities for social inclusion of elderly people and persons with disabilities. The specific objectives of the project are related to building a successful model for patronage care for elderly and disabled people.
