In recent years, the health-demographic situation of the population in the Republic of Bulgaria has been characterised by ageing, chronic morbidity and disability, which leads to an increase in the cost of treatment and at the same time hinders access to medical care and quality healthcare services. The shortage of adequate healthcare facilities for healthcare and permanent care, as well as the limited financial capacity of adults, people with disabilities and their families, combined with the high relative share of poor household members made up of one person above 65/54 % in 2017/strikes the need to introduce alternative methods and implement a comprehensive approach to meeting the health and social needs of the population. Such an alternative is the service “Patronage care for adults and persons with disabilities”. The project proposal of Varna Municipality aims to improve the quality of life and reduce the social exclusion of older people over 65 years, with restrictions or inability to self-service, as well as people with disabilities and their families through: •Building capacity for providing the service “Patronage care for adults and persons with disabilities” on the territory of Varna Municipality and expanding the network of accessible and quality services in the community and in the home environment, in line with the National Strategy for Long-term Care and the Action Plan for the period 2018-2021;•Maintenance of care at home; •Provision of hourly mobile integrated health and social services; •Consulting users and their relatives on medico-social problems; • Maintain good relationships with general practitioners and exchange of information on the condition of users.Result of the realisation of planned activities, not least the number of elderly residents, and the reduction of the number of hospitality, prevention and prevention.
