This project proposal aims to enable independent and dignified living of the elderly and people with disabilities on the territory of the municipality of Gabrovo by providing quality, accessible and effective cross-sectoral long-term care services tailored to the real needs of the target groups. The identified target groups cover elderly people over 65 years of age with restrictions or inability to self-service; persons with disabilities, including with various forms of dementia, and their families; adults, including persons with disabilities, placed in specialised institutions and their families; employees of social and health service providers. The project will provide support to the target groups through the opening of 2 social services of resident type — Care Centre for persons with different forms of dementia and Centre for care for elderly people, incapable of self-service on the territory of Gabrovo municipality. The two centers will be based in the four floors of the building of the former House for Medical and Social Care for Children — Gabrovo, each of them with a capacity of 15 seats. The implementation of the project will last 16 months and will start with activities related to the development of rules for the regulation of services, assignment of services to a social service provider, hiring and training of staff, provision of services. An external contractor will also be selected to supply essential equipment and furnishings for both centres. The provision of the two services will be implemented for a period of 12 months and will meet the needs of 30 users. As a result, the implementation of the project will build capacity to provide resident care for persons with disabilities and elderly people dependent on the care of the target groups of the project.
