With the implementation of the project proposal aims to create a sustainable model for the provision of integrated health-social services for people with disabilities and their families and elderly people over 65 years old, at the risk of self-service and at risk of social exclusion.The project will provide part-time integrated services, either individually or in combination, including health care activities and social support activities in a home environment. A project management team will be set up, which will be responsible for the implementation of all planned activities in a qualitative and timely manner. The team will start its activity with an information campaign on the objectives and nature of the project and the conditions for use of the services. A team will be appointed to manage the activities of the project, which will be organised according to the integrated services. nurses, for the provision of health care and medical surveillance according to the needs of the persons — providing medical treatment, etc.; domestic orderlies to carry out the following activities: assistance to consumers in carrying out personal and household hygiene, assistance in food preparation, shopping and assistance in the use of administrative services, etc. Training for staff providing the services, according to the program developed by the Ministry of Health, as well as supervision will be provided. In case of an established need, users will be given the opportunity for rehabilitation services. To overcome loneliness, social isolation, as well as to receive support for independent living, psychological services will also be provided in case of a stated need. For the purposes of the project, construction and installation activities will be carried out on a premises, which is municipal property, designated as an office of the service management team and adjacent bathrooms.
