The following needs will be addressed for: organised social services corresponding to the growing local demand, medical and rehabilitation services aimed at elderly and disabled people in Dolni Chiflik municipality; multiplication of good practices and promotion of a culture of consumption of social services, as well as a culture of behaviour towards socially vulnerable groups in need by building a model of patronage care, 52 persons from the target groups will be included: older people over 65 with restrictions or inability to self-service-people with disabilities and their families General objective: Improving the quality of life and opportunities for social inclusion of people with disabilities and the elderly in the Dolni Chiflik municipality, by providing a network of services in the home environment and building the appropriate capacity to provide themSpecific model of patronage care for elderly and disabled personsProviding integrated medical care and social services at home, psychological support and counselling oriented to prevent loneliness and isolation, adapted to the basic needs of users, to be independent and to stay as long as possible in their usual environment Results: Technical Assurance of servicesTrained and superseded staff to the services of graduates of multi-stakeholders and cross-carriage services provided by educating services to differentials of multi-students, and meeting the needs of graduates in the cross-sectional environment, the service’s designed to provide cross-carriage services and cross-sectional services to the service-carried out of multi-carriage services and the establishment of cross-carried-up services.
