The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the municipality, by providing a network of services in the home environment and building appropriate material and personnel capacity for their provision by introducing a model for patronage care for elderly people and persons with disabilities, including chronic diseases and permanent disabilities to provide hourly mobile integrated health-social services in their homes.
