Establishment of a Unit for hourly provision of services for social inclusion in the community or in a home environment in Karlovo Municipality. Through the implementation of the project proposal, integrated services will be provided for people with inability to self-service and for people with disabilities, combining complex actions towards long-term care, including social health services in the community or in the home environment. Project target group: 1. People with disabilities and their families. 2. People over 65 years old with restrictions or inability to self-service.The project will be implemented through activities and measures, which will be implemented in partnership with a social service provider organisation SONIC START Ltd. in order to achieve the expected results in the project proposal as follows: 1. Organisation and management of the project. Selection and matching of users and social service providers. 2. Provision of hourly social services in the community and in the home environment. Ensuring access to integrated services, according to the specific needs of disadvantaged people on the basis of an individual social assessment, by supporting the activities of the Unit for hourly provision of services.3. Training of staff in the provision of services.4. Provision of psychological support and/or counselling and/or supervision of staff providing social services.5. Motivational and/or psychological or other type of support to service users according to their individual need.6. Information and publicity. As a result of the implementation of the activities, the quality of life of adults and persons with disabilities will be improved by creating conditions and supporting the effective exercise of their right to independent life and social inclusion, while respecting their rights, taking into account their capabilities and specific needs.
