Patronage care for elderly and disabled people in the municipality of Knezha

The project envisages building a model for patronage care for elderly people and persons with disabilities, including chronic diseases and permanent disabilities, with the aim of providing part-time health and social services in their homes. Patronage care is not included in the list of state-delegated activities financed by the state budget. The municipality of Knezha maintains part of the services as a local activity, but the capacity of these local activities cannot meet the needs of those in need. The activities under this project are aimed entirely at elderly people over 65 with limitations or inability to self-service, as well as people with disabilities and their families, people who have no relatives and for whom there has been a social need for hourly services. A large proportion of people over 65 who have retained their ability to live independently suffer from serious accompanying health problems. There is a high percentage of people with disabilities in the municipality for whom there is no adequate support. The project provides a set of social services focused on providing care and support in a home environment. Elderly people with chronic diseases and permanent disabilities will be provided with specialised medical care in their homes. The mobile service includes a variety of social, health, administrative, household activities, as well as activities aimed at improving the quality of life of elderly people and persons with disabilities, depending on the specific condition and constraints. The services will be provided on an hourly or on request basis to 50 users who are informed and willing to participate in needs assessment and support planning. Specific activities are aimed at stimulating consumer activity, health care, limiting risks in the home and the risk of dependence on institutional care.
