The project proposal includes activities through which the beneficiary will provide the social service “House assistant” and the health service “Medical rehabilitation”. Users of the services will also be assisted by a consultant who will hold talks, give recommendations and consultations on topics relevant to the target groups. The assistant will develop and the organisation and management team will disseminate material to the population and target groups on the principles of healthy lifestyles and health problems of vulnerable groups. For this purpose, the persons who will benefit from the social service will be selected; there will be a selection of those who wish to provide it by working in the post of Domestic Sanitary, a person will be appointed to the post of Collaborator, Social Affairs, who will coordinate the work of the staff with the users. The innovative element that will improve users’ access to health services in the different localities of the municipality, including remote ones, consists of providing medical rehabilitation in the home of consumers. So far, this type of home healthcare service has not been provided on the territory of the municipality. For this purpose a rehabilitator will be appointed. Target groups:1.People with disabilities and their families;2.People over 65 years old, unable to self-service.In order to achieve the objectives of the project, the following activities are planned:1. Selection and recruitment of project 2 staff. Provision of a supportive social service “Home Assistant” 3. Provision of health service “Medical rehabilitation” and health care activities through targeted actions.4. Providing information to the population about the principles of healthy lifestyle and health-related problems.The implementation of the project will significantly contribute to improving the access of people from the target group from all settlements on the territory of the municipality to health and social services.
