In the modern conditions of health-demographic and economic crisis, combined with ongoing reform and inefficient health system, the need to introduce alternative methods for meeting the health and social needs of the population in the country and respectively in the municipality of Pazardzhik is exacerbated. Such an alternative is Patronage Care. The project proposal “Services in the House” will provide hourly mobile health services, social services, psychological support and counselling in the homes of the target group in 32 settlements in the Municipality of Pazardzhik. The project “Services in the Home” builds on the model for provision of hourly mobile integrated health services — social services provided by Home Social Patronage and the project “Independent Life”. Induction and supportive training of the persons who will provide the services is envisaged. The project will cover 213 targeted persons. The services will be provided 12 months, up to 2 hours per day for one person. They will be provided by social workers, medical professionals, domestic workers and psychologists. A six-person Project Management Team will be formed. A Service Management Team will be appointed through a selection of six people. To promote the project, two information campaigns will be carried out at the start of the project and one final one. A sign will be drawn up, which will be placed on the door of the project office. A notice will be published on the municipality’s website. Leaflets will be distributed in accessible places in the municipality of Pazardzhik, Labour Office, Social Assistance Directorate, town halls, NGOs, social service providers, etc. To move the staff providing services in remote and hard-to-reach places a car will be purchased. A driver will be appointed. Three computer configurations will be purchased — computer, monitor and keyboard and mouse, three multifunction devices three in one- printer, scanner and copy drive and one laptop.
