The project proposal is in line with the specific objectives set under the procedure “Patronage care for elderly people and persons with disabilities”. The project envisages activities aimed at a decentralised model for the provision of services in a family environment. The aim of this project proposal is to expand the opportunities for improving the quality of life of people with disabilities from the Municipality of Radomir, their motivation for independent and independent lifestyle. Integrated social-health services in a home environment will be launched for the purposes of the project. In addition to home care, community services will be developed to overcome loneliness and isolation. Specialised medical care in their homes will be provided for people with chronic diseases and permanent disabilities. This innovative social service in the community for the municipality of Radomir will be based in the town of Radomir for a period of 12 months, according to a methodology developed by the Ministry of Health.The project also foresees the purchase of a means of transport, which will ensure mobility of the staff to the users of services. With the realisation of the project will be the opportunity to upgrade the already existing social services and the provision of new ones. Older people, single people and people with disabilities will be able to use hourly mobile integrated social-health services in their homes.
