The project was implemented by the Municipality of Jászapáti. The areas of supply and operation of the practice community are Jászapáti, Jászkisér and Jászivány in the district of Jászapáti. Jászapáti district is 290/2014. (XI. 26) on the basis of the classification of the beneficiary districts, districts to be developed with a complex programme. The total population of the district on 1 January 2015 is 31 837 (based on the TeIR data source). The overall objective of our project is to improve the health status of the population, to increase life expectancy at birth and to the number of healthy life years, and to contribute to re-orientating primary care with a preventive focus, thereby increasing the health awareness of the population and reducing territorial inequalities related to prevention. Milestones: — To improve the coordination of primary health care and public health tasks by combining a disease-centric approach to health care and a health-centric approach to public health; — The provision of extended primary care services within the new framework of cooperation between the actors of primary care; — To strengthen the preventive activities of the basic supply system by expanding the preventive services provided by the practices and by integrating cooperative models of practices into the supply system; — Within the framework of primary care competence, practices should be able to provide definitive care to patients as widely as possible and to provide the population with a higher level of progressivity and more costly care for the general public on a continuous basis; — Improving access to patient care and thus reducing territorial disparities resulting from unfilled practices. Within the framework of the project, 4 general practitioners and 1 pediatrician will establish a community of practice for the localities in which they operate. In addition to the professional leader, the practitioners, 5 main practice nurses, 1-1 person dietist, physiotherapist, health officer, public health coordinator, nursing assistant, praxis community manager and 2 protective women will be employed to implement the project. The target group is the population subject to the territorial care obligation of the supported practice communities, the professionals working in the extended primary care, the prospective practice communities, the outpatient outpatient institutions, health promotion offices, social services, home care providers and the staff of public pharmacies operating in the area. The planned development fits with the measures concerning the equal opportunities target groups identified in the Local Equal Opportunities Programmes, and has been explained in detail in the technical plan. The organisation applying for funding has the necessary professional experience to achieve the objectives of the call, as well as the human resources and infrastructural conditions necessary to achieve the objectives of the application. The organisational conditions required for the implementation of the project are in place. The objectives of the project are in line with the policy environment and the specific objectives set out in the call. Our current project is in close synergy with the Basic Care — Development Model Programme, it is based on it, and in many points the activities of the core programme will be carried forward to the present call, adapting the implementation to professional activities. The planned activities contribute to the achievement of quantifiable results and the target values of the indicators committed are set in the minimum number of the calls. Horizontal aspects are applied during the implementation of the project, i.e. equal opportunities legislation will be complied with. Environmental sustainability aspects are applied. We have concluded a cooperation agreement with the existing Jászberény Health Promotion Office. The Community of Practices has undertaken to provide services to the population of unfilled practices in relation to the activities of the practice. In the practice community, we undertake to employ a public health coordinator with qualifications and qualifications as a qualified public health professional in master’s degree. Interventions contribute to the prevention of chronic diseases through awareness-raising, dissemination of screening and preventive preparation and capacity building of the healthcare system. Interventions contribute to reducing disparities in territorial availability of services. The outsourcing activities were planned for 2.3 % of the costs.
