CASCADE: Community Areas of Sustainable Care and Dementia Excellence in Europe

Cross-border Collaboration Enhances Dementia Care in the 2 Seas Region

Overview: The CASCADE project is a cross-border initiative aimed at revolutionizing dementia care within the 2 Seas area, encompassing regions in the UK, France, Belgium, and the Netherlands. By developing flexible care models and integrating innovative technologies, CASCADE seeks to enable individuals with dementia to maintain independence and reside in their homes longer, thereby reducing the strain on hospital resources.

Objectives:

  1. Develop Flexible Care Models: Introduce adaptable care solutions that allow seamless transitions between home-based care, specialized guesthouses, residential facilities, and hospitals.
  2. Implement Innovative Technologies: Deploy telemedicine and residential monitoring systems to support independent living and enhance decision-making for care providers.
  3. Establish Centers of Excellence: Create hubs dedicated to dementia care, offering comprehensive services, training, and development opportunities.

Key Features:

  • Telemedicine Systems: Utilization of video communication tools to facilitate interactions between patients and healthcare providers, minimizing unnecessary hospital visits.
  • Residential Monitoring: Implementation of systems that assist individuals in living independently while providing critical data to caregivers.
  • Holistic Care Approaches: Personalized care plans developed through collaboration among patients, families, and healthcare professionals.

Target Audience:

  • Individuals with Dementia: Persons seeking to maintain independence and quality of life.
  • Caregivers and Families: Those involved in supporting individuals with dementia.
  • Healthcare Professionals: Providers aiming to enhance dementia care practices.

Impact:

  • Increased Care Capacity: Introduction of over 50 new beds to accommodate patients within the region.
  • Enhanced Patient Independence: Adoption of technologies and care models that support individuals in remaining at home longer.
  • Community Engagement: Initiatives such as World Café events foster collaboration among stakeholders to improve care strategies.
