Social Services Centre in the municipality of Dębica — a new quality of life for residents

Project implemented in the Municipality of Dębica — creation of CUS through the transformation of GOPS. The project results from the need to improve the quality and scope of the catalogue of social services (US), adapt US to the needs and changing reality, participatory model of U.S. implementation and de-institutionalisation of them (inhabitants of municipal villages have difficult access to the US). Recipients: general residents of Gm. Dębica, employees of UG Dębica, CUS and subordinate units, representatives of NGOs. CUS ensured coordination of health and health services, cooperation with employment services (agreements, cross-sectoral cooperation). CUS Director, Social Services Organiser, 3 Coordinators of Individual Social Services Plans (KIPUS), Local Community Organiser and specialists providing substantive services.The project is implemented in 4 stages (milestones (KM). Implementation in accordance with the principles of comprehensiveness, universality, subjectivity, strengthening social ties, quality: 1. Diagnosis and Implementation Plan of CUS (PW CUS) 2. Implementation of the structure and implementation of social services.3. Update of PW CUS 4. Report- Review and evaluation of CUS implementationThe implementation of services resulting from diagnosis in 1KM will be subject to constant monitoring and may changeU.S. implementations are new or of increased scope and will be carried out outside the headquarters of CUS, on the territory of the villages. My ass. 1= 1 hp, implementation of PW CUS in 2022 is 2 hp. 3 KM will start updating PW CUS based on analysis of monitoring data (Zad.3) and further implementation of services according to the updated PW CUS. 4 KM- review and evaluation of the CUS PW (ref. 4). Implementation of social services — from KM 2 to the end of the project.The implementation of the project (including management and promotion) takes into account the principles of equality and accessibility and sustainable development.In order to ensure high quality of services, an appropriate number of KIPUS will be employed (3); in addition, 1 ON in the dimension of at least 0.5etat.The duration of the project implementation: 01.07.2021-31.11.2023 (29 months)Amount of co-financing: 3 108 565,00 zł
