The project is a response to a health problem occurring in the Lower Silesian Voivodeship, which is diabetes (type 2). The project aims at preventing and early detection of type 2 diabetes among residents of the Lower Silesian Voivodeship. The main objective of the project is to reduce the number of cases of type 2 diabetes among 6.020 inhabitants (3.311K, 2.709M) of the Wrocław subregion and Wrocław through their participation in educational activities, prophylactic studies and consultations on the detection of type 2 diabetes — the target will be achieved by 30 June 2023. The planned tasks are consistent with the Regional Health Programme for Preventing and Early Detection of Type 2 Diabetes in Dolnośląskie Province and include: Stage 1 — Information and education activities, Stage 2 — diagnostic activities and Stage 3 — consultation meetings (Stage 4 — evaluation of the effectiveness and achievement of the objectives of the Programme, will be conducted by the applicant through surveys collected from project participants and measures of effectiveness of the programme implementation. The data will be collected throughout the duration of the project by the applicant). The project is aimed at a minimum of 6.020 inhabitants of the Lower Silesian Voivodeship (subregion of Wrocław and Wrocław), who are aged 35-64 and do not have diagnosed type 2 diabetes in the previous interview (an additional criterion for women: women who are not pregnant). The assumed result indicator is: Number of people who, thanks to ESF intervention, applied for a preventive study [6.020]. The project also provides for the possibility of providing support during the afternoon and evening hours and on Saturdays.
