The aim of the project is to support the digitalisation of healthcare providers providing guaranteed benefits such as primary health care in at least the scope of the “provision of the POZ doctor” by: — expansion of the IT systems of POZ facilities in order to enable the provision of public e-services and integration with the e-health system together with the provision of ICT infrastructure necessary for the provision of e-services and integration with the e-health system. The project will be implemented only in entities with a contract for the provision of health care services financed from public funds — a healthcare provider providing guaranteed benefits such as basic health care in at least the scope of the “provision of a doctor’s POZ”. Establishments will be selected as part of a selection procedure open to all interested POZ outlets meeting the selection criteria. Under the project, support will be granted in the form of a grant based on Article 35 of the Act on the principles of implementation of cohesion policy programmes financed under the 2014-2020 financial perspective. There will be two envelopes for the selection: Covering 70 % of the allocation allocated to the project — foreseen for municipalities with a lower than average income per capita of the municipality in Poland; 2. covering 30 % of the above-mentioned allocation — provided for municipalities with a higher than average income per capita of the municipality in Poland. Each envelope will have the order of submission until the available allocation is exhausted. If the allocation is exhausted within one envelope, it will be possible to transfer funds between envelopes.
