The subject of the project is to take measures to improve the health of people at risk of poverty and social exclusion from the municipalities of Korfantów, Łambinowice and Tułowice: the elderly and the disabled. As part of the project, health services will be provided in the daily medical care centres for the elderly established in the Opole Rehabilitation Center. The participants of the project will have the opportunity to use the rental of rehabilitation equipment retrofitted as part of the already completed project and the project underway. In the project, people with a medical indication directly or up to six months after hospitalisation will be preferred, meeting the conditions of recruitment. In the daily form of stay, 4 places of care will be created. The project responds to perceived social problems: accelerating the ageing of the population (an increase in the number of single-person households of the elderly living with difficult access to health services) and the problem of increasing the economic burden of families, — the difficulty or impossibility of performing basic life activities independently (according to the Barthel scale) and the need to take measures to improve the number of disabled people, especially at the age of 65+, — a small scope of safeguarding the needs of older residents in terms of places of residence in daily homes with a systematic increase in their number. As a result of the project, nursing and medical assistance will be intensified, as a continuation of hospital or outpatient treatment, including rehabilitation and physiotherapy, occupational therapy, activities leading to the improvement and improvement of the comfort of life of these people. Participants of the Project will have free, comprehensive health services, transport to the place where health services are performed, food.
