The project will be implemented in the district of Nysa and will cover 20 people (K=10; M=10) which due to old age, including disability, require care or support due to the inability to perform at least one of the basic daily activities on their own. The aim of the project is to improve the quality of life and functioning of people at risk of social exclusion, as well as deinstitutionalisation of medical care for people in need of support in everyday functioning due to old age, including people with disabilities. The project assumes the deinstitutionalisation of medical care through the provision of care and health services at the place of residence, telecare (purchase for the participants of the project of specialised medical bands together with 24/7 contact with paramedics), as well as complementary support in the form of physiotherapy, speech therapy and psychiatrist doctor.
