The aim of the project is to take measures to improve the health of people at risk of poverty and social exclusion, the elderly and the disabled from municipalities: Korfant, Łambinowice, Tułowice. The project will provide health services for the elderly in the place of residence and support for elderly people, people requiring support in everyday functioning as a replacement for actual caregivers and respite care in the form of an outgoing health program. The project will prefer people who require support in col. functioning with a medical indication (medical qualification or overall geriatric assessment — COG directly or up to half a year after hospitalisation) who meet the conditions of recruitment in accordance with the conditions of recruitment 8.1. The project responds to perceived social problems: the accelerated ageing process (which translates into an increase in the number of 1-person households of single people living with difficult access to health services) and the problem of increasing the economic burden of families, — the difficulty or inability to perform basic life activities independently (according to the Barthel scale) and the need to take improvement measures in disabled, dependent people, especially at the age of 65+. As a result of the project, nursing and medical support will be intensified, as a continuation of hospital or outpatient treatment, rehabilitation and physiotherapy, as well as additional activities leading to the restoration of activity and increasing the comfort of the participants’ lives. Participants of the Project will have free comprehensive health services, transport, accommodation, meals. The result of the project will be the deinstitutionalisation of health services to the local community, in particular the inhabitants of rural areas, where there is a problem of accessibility to health services.
