Transformation of the care system for Praguens, who need intensive support and have been placed in facilities outside the HMP, including strengthening of care capacities in Prague

The project is being developed as an implementation tool for managing change in providing sufficient care and support capacity for Praguens in Prague, improving coordination of care and support, ending the practice of placement of Praguens outside of Prague and the overall transformation of so-called institutional care.
