Digital services for health pathways and their coordination (Pépites-Parcours)

The operation “Digital services for health pathways and their coordination (Pépites-Parcours)” is part of the Health System Transformation Strategy, designated by “MA SANTE 2022”, which aims to stimulate a better organisation of professionals for the benefit of patients. With the implementation of care pathways, it will no longer be up to the patient to make the link between the different health professionals, but to the caregivers to coordinate. To do this, a range of digital services allowing the exchange and sharing of information around the patient-user will be offered to professionals in the health, medical, social and social sectors, and deployed throughout Guyana. In a logic of decompartmentalisation, professionals in the health, medico-social and social sectors will soon be able to benefit from a range of digital services for the care, sometimes complex, of their patients or their users. Below are some examples of the tools that will be available: — Coordination tools – Directory and identification of the care team – Development of a PPCS (personalised health coordination plan) – Tools for screening, tracing and assessing the patient’s situation – Liaison book, event notifications, secure chat
